Propolis + Zinc + Honey + Pelargonium sidoides: An Adjunctive Approach for Acute Throat Discomfort in Children

The results showed that adding the combination supplement to conventional treatment significantly improved sore throat and pharyngeal erythema scores on Day 6 compared with standard therapy alone (p<0.001 and p<0.05, respectively); dysphagia scores improved significantly by Day 4 (p<0.01); and total tonsillitis severity scores were significantly better than the control group on both Day 4 and Day 6 (p<0.05 and p<0.001, respectively). Only one patient in the standard-therapy-only group was deemed a treatment failure due to the need for ibuprofen, and no adverse events were reported during the study period.
The significance of this study lies in that it provides randomized controlled evidence for the "adjunctive" value of this specific formulation in a real pediatric population, rather than proving it has a curative effect on the infectious disease itself.

Pelargonium sidoides extract (commonly used as the standardized preparation EPs 7630) has an independent systematic review foundation in the area of acute respiratory tract infections. A 2013 Cochrane systematic review by Timmer et al. pooled multiple randomized controlled trials to evaluate the efficacy and safety of Pelargonium sidoides extract for treating acute respiratory tract infections in children and adults. The review included adult trials of acute bronchitis (3 trials, 746 patients) and pediatric trials (3 trials, 819 children), both showing significant heterogeneity — with dosage form being one potential cause: liquid formulations showed efficacy, while tablet formulations did not. For sinusitis, one unpublished study (103 adult patients) showed a hazard ratio of 0.43 for complete resolution by Day 21 (95% CI: 0.30–0.62), but the quality of evidence was rated as "very low." For the common cold, one study showed efficacy by Day 10 but no significant effect by Day 5. The authors concluded that Pelargonium sidoides extract may help relieve symptoms of acute sinusitis and the common cold in adults, but with uncertainty; it may help relieve symptoms of acute bronchitis in adults and children and sinusitis in adults, but reliable data for other acute respiratory infections have not been found.
- Zinc: Immune Homeostasis Regulation
Zinc participates in immune homeostasis regulation and is a cofactor for multiple enzyme systems. In the common cold domain, a 2024 Cochrane systematic review by Nault et al. noted that zinc supplementation may have little to no effect on preventing colds, but may shorten the duration of existing colds, accompanied by an increase in non-serious adverse events. In terms of evidence strength, this combination is currently supported by only one moderately sized, open-label randomized controlled study (Cardinale et al., 2024), and larger-scale, double-blind, long-term follow-up confirmatory trials are still lacking; the evidence for Pelargonium sidoides extract itself (Timmer et al., 2013) is also of low quality and affected by dosage form. Therefore, stating that it "can serve as an adjunct to standard therapy and may help improve symptoms and shorten the duration of discomfort" is a prudent framing, while any claim of "preventing respiratory infections" or "replacing medication" goes beyond the scope of existing evidence.
IV. Practical Tips and Safety Reminders for Parents
At the practical level for parents, this supplement cannot replace the standardized evaluation and necessary anti-infective treatment required for pediatric acute pharyngitis. When purchasing and using, confirm the applicable age, pay special attention to the fact that infants under 1 year of age are contraindicated for honey-containing preparations, and follow the recommended daily dosage.
If a child exhibits any of the following conditions, seek medical attention immediately rather than relying on dietary supplements:
- Persistent high fever that does not subside
- Difficulty swallowing or breathing
- Obvious purulent tonsillar exudate
- Recurrent or worsening symptoms
V. Conclusion
The combination of "honey + propolis + Pelargonium sidoides extract + zinc" shows certain potential for adjunctive symptom improvement in pediatric acute tonsillopharyngitis, but the existing evidence is still preliminary — the study design was open-label with a moderate sample size, and confirmatory value is limited. Positioning it as "an adjunctive option on the basis of standard therapy," used rationally under the guidance of a healthcare professional, is currently the most prudent approach.
